Karen Skoog & Raelene Eckert
Care Managers
Karen Skoog, RN, BSN. Karen has been working in the field of care management for over 14 years. After graduating from the University Of Oregon School Of Nursing in 1975, she worked in Intensive Care, Coronary Care and Kidney Dialysis. After raising her family she refocused her career into Case Management and Geriatric Care Management.
Karen is especially interested in families in crisis as they deal with chronic illnesses and neurological conditions such as Alzheimer's Disease. Karen works with clients and their families as they face aging and/or health issues and assists them in collaborating with the appropriate health and professional services to create a plan that meets their needs and provides enhanced quality of life.
Raelene Eckert, R.N. Raelene graduated from Illinois Masonic School of Nursing in 1979. She is a registered nurse in Oregon and Washington states. Her nursing experiences include Coronary Intensive Care, Cardiac Rehabilitation, Allergy, Asthma and Rheumatology. For the past six years, she has focused on geriatrics and gerontology.
After her experience as a long distance caregiver for her aging parents, it became quite apparent to her that our fragmented
health care system was difficult to navigate for elders and their families. It is often confusing, and lacking in the continuity and follow-up care seniors require to maintain their health, safety and
independence.
Raelene's passion is to help clients determine the best plan of action for their individual health circumstances and collaborate with family and other professionals in its implementation.
Raelene provides health and wellness education to assist her clients in finding personlaized solutions that will help maintain their independence and quality of life.
